South Niagara Health & Wellness Centre is moving to its new permanent location at the state-of-the-art Gilmore Lodge in Fort Erie. Services at the new location will begin on January 6, 2025, with the move occurring on December 30, 2024.
Currently operating at a temporary location at 234 Bunting Road in St. Catharines, the Centre’s relocation to Gilmore Lodge will see the continuation of high quality care delivered to patients in a brand new, innovative facility. Gilmore Lodge, located at 60 King Street in Fort Erie, is a modern, three-story space designed to integrate a variety of community services in a welcoming, accessible environment.
The South Niagara Health & Wellness Centre, a collaborative program by Niagara Region, Hotel Dieu Shaver, and March of Dimes, provides individualized care for older adults recently discharged from hospital or at risk of hospitalization. Treatment is structured to enhance mobility, function, and independence through a combination of physiotherapy, speech-language pathology, wellness programming, and health education. Participants also benefit from connections to community resources to support long-term independence.
To access the South Niagara Health & Wellness Centre’s services, a referral form is required. Forms can be found on our website, and eligible participants must meet the outlined criteria.
