Patients are admitted to this program from an acute care hospital after an acute event.
This program consists of a time-limited, low-intensity interprofessional rehab plan of care. This low-intensity program is delivered through a combined and coordinated use of medical, nursing and allied health professional skills. The goal of this program is to improve functional ability and independence to facilitate a safe transition to the community.
Rehab Intensity: A coordinated Interprofessional care plan that can include up to two hours of rehabilitation per day and is determined by a person’s tolerance.
